About the Patient Safety Authority (PSA)
Event reporting provides many opportunities to improve patient safety and quality.
Pennsylvania is the only state that requires healthcare facilities to report all events, including those that cause harm or potential for harm. The PSA receives these reports via the Pennsylvania Patient Safety Reporting System (PA-PSRS), one of the largest patient safety databases in the world with over 4 million reports.
The PSA analyzes PA-PSRS reports to prevent recurrence and shares that information through multiple channels, including eLearning.
